sick as a dog, to be To feel very ill, particularly to feel nauseated. Despite being touted as mans best friend, dogs are often maligned. This proverbial simile dates from the sixteenth century or earlier and is still current.

"Sick as a dog," usually means "extremely sick" and dates back to at least the 17th century, it is simply descriptive phrase. "Being sick" in Britain means "to vomit." And dogs are known by their frequent vomiting, you can see connection now.
